Insurance Benefit Limits

  • Please note that benefits for Durable Medical Equipment may be limited by your health plan, require prior authorization, or there may be a co-pay or co-insurance amount to satisfy.

  • There are some services that may not be covered by your insurance carrier.

  • It is YOUR responsibility to understand your health plan benefits and to obtain any required prior authorization to cover the cost of your care. Please contact the Member Customer Service by calling the number on the back of your insurance card.

Pre-Payment Policy

  • You will be required to make a prepayment for your service on the day of your visit.

  • Forms of payment accepted include cash, check, and all major credit cards.

  • Our Finance Company is Care Credit. Please click this button for additional information.

Important

  • If you have new health insurance or different coverage than your last visit, please notify us so that we properly bill your insurance for your medical care.

  • Always bring your insurance card(s) to your appointments.
